Who is to blame for the Hammersmith Bridge saga?

Sarah Olney writes in New Civil Engineer:

Not a day goes by where I don’t receive an email from a despondent constituent, justifiably angry with the ongoing saga at Hammersmith Bridge and rightly demanding answers.

These are adults who have found their journeys to work disrupted, children who have been forced to take longer, often more dangerous routes to school and emergency service drivers unable to perform their jobs with the speed of access they require.

So, understandably, when I am contacted, the question if often a case of, who? Who is to blame for this farcical situation and who can resolve it?
